When Duane Bradley arrives in New York City and checks into the sleazy Hotel Broslin, he is repeatedly asked one question: What’s in the basket? Few would guess that the answer would be his hideously deformed twin brother Belial… Separated at birth, the two brothers are now out for revenge against the doctors who left Belial for dead and will stop at nothing on their vengeful path. When Duane strikes up a relationship with a doctor’s receptionist, however, Belial’s jealousy and rage threaten to erupt out of control, setting the scene for a gore-drenched finale.

Picking up straight after the events of the first film, Duane and his basket-dwelling brother Belial are taken to the city hospital. With their secret life unmasked and their cover blown, the brothers unwittingly become media celebrities with a pack of journalists on their trail, which attracts the attention of Duane and Belial’s long-lost aunt, affectionately known as Granny Ruth. After springing them from the hospital, Granny Ruth takes the brothers under her wing at her mansion home, which doubles as a safe-haven for an assortment of freaks and oddities who have sought shelter from the world. Before long, however, the duo’s whereabouts are discovered and they must join forces with Granny Ruth and her army of mutants to get rid of the tabloid hacks once and for all.
